Transforming Healthcare Delivery: The Role of Payment Models in Supporting Interprofessional Team-based Care

In the evolving healthcare landscape, the methods of financing healthcare services play a crucial role in shaping how care is delivered. A key transformation needed to improve patient outcomes is the shift from fee-for-service models to value-based care. This change aims to enhance patient outcomes, optimize resources, and ensure healthcare sustainability. Central to this transition is aligning payment models with interprofessional team-based care, which emphasizes collaboration among healthcare professionals. Interprofessional care has been shown to improve outcomes and efficiency, yet traditional payment systems often fail to support these approaches, focusing on individual, billable services rather than fostering holistic, team-oriented care.Implementation: In this oral presentation, I will explore various healthcare payment models and their impact on interprofessional team-based care. The discussion will highlight how certain payment models can either enhance or limit collaboration among healthcare providers. Special attention will be given to value-based payment models, which incentivize high-quality, patient-centered care and improved outcomes by shifting financial risk from patients and payors to providers. This incentivization encourages a preventative, proactive approach to care that aligns with the goals of interprofessional teams.Evaluation plan: To evaluate the effectiveness of these payment models, I will present case studies and examples that illustrate real-world outcomes. The evaluation will focus on key indicators such as patient satisfaction, resource utilization, and long-term health improvements. Additionally, I will discuss the sustainability of these models, with an emphasis on their ability to improve access to care, reduce disparities, and promote equity.Outcome(s) and significance: The main goal of this presentation is to offer insights into how payment models can better support integrated, patient-centered care. By framing payment reform as an investment in improving population health, rather than a short-term cost-reduction strategy, I hope to influence policymakers and healthcare leaders to prioritize long-term, sustainable solutions for a more equitable healthcare system.
