WA State's Community Hub- Creating a Diverse Interprofessional Workforce for Social Care Network(s)

  • Expanding Interprofessional Health and Social Care Teams and Collaborative Practice

This presentation will showcase WA state’s Accountable Community of Health (ACHs) and their specific role in serving as Community Hubs for community-based care coordination and delivery of Health-Related Social Needs (HRSN) services. ACH’s are a key strategy of the Medicaid Transformation Project 2.0 in partnership with the Washington State Health Care Authority. The Hub currently uses the care coordination system (CCS) as the client management system to screen and assign individuals to the right partners.

The Community Hub helps individuals and families in our South-Central Region more easily connect to supports and resources to achieve their optimal health and wellbeing through improved navigation of the health and social services systems, access to culturally responsive services for communities by people in their communities, improved coordination across sectors, improved advocacy for resource and access needs through the availability of robust data, and large-scale community-based workforce support.

Greater Health Now’s (GHN) GHN ACH has partnered with a network of organizations to provide community-based care and ensure that every client's unique needs are met. GHN has a no-wrong-door approach, meaning anyone living in Washington can refer themselves or others to our Hub for assistance.

The target audience for this session is geared towards mid to advance level practitioners, academia and those assuring the integration of current health policy into educational programming and clinical practice.

Attendees will have the opportunity to identify strategies to engage community health systems and professionals in their local communities to advance partnerships and collaboration advancing and addressing the Health Related Social Needs of their communities.

Learning Objectives

  • Participants will be able to describe the function and significance of ACHs within Washington State's Medicaid Transformation Project 2.0, focusing on Health-Related Social Needs (HRSN) services and coordinating community-based care.
  • Recognize opportunities to enhance interprofessional collaborative practice through a Social Care Network lens.
  • Participants will gain insights into how to engage with local community health systems and organizations to strengthen partnerships,
    support diverse health workforce development while addressing Health Related Social Needs.
